Group One - Gastrointestinal
Section A
Distress from fats or greasy foods, such as nausea, dizziness, headaches, etc.
Distress from onions, cabbage, radishes, cucumbers, (cause bloating, gas, etc,)
Stool Appears yellow, clay -colored; foul odored; shows undigested foods.
Skin is grayish, pasty, oily on nose and forehead.
Have had jaundice,hepatitis.
Bad Breath, bad taste in mouth, body odor (including feet).
Unusual redness of palms of hands.
Unaccountable burning of soles of feet.
Varicose veins, hemorrhoids, phlebitis,veins showing on chest or stomach area.

Indigestion occurs 2 to 3 hours after meals, fullness, bloating, sourness, etc.
Heavy, full loggy feeling after eating heavy meat meal.

Excessive lower bowel gas.

History of constipation which alternates with diarrhea.
Section C
Stomach pain occurs 5 or 6 hours after eating, usually at night, relieved by eating or drinking milk or cream.
Above symptoms flare up in Spring or Fall of year.
Have been told you have stomach ulcers.
Above symptoms aggravated by worry and tension; relieved by vacationing.

Diarrhea occurs frequently.
Diarrhea occurs without apparent cause, resistant to ordinary treatment.

Indigestion occurs immediately after eating.

Group Two - Functional Nervous Indications
Section A
Eyes are sensitive to bright lights.
Have tightness in throat
Form gooseflesh easily, sweat without temperature rise, "cold sweats".
Voice rises to high pitch or is lost during stress, such as arguments, public appreaness.
Easily shaken up, easily startled, heart pounds hard from unexpected noise.
Prefer being alone, uneasy when "center of attraction".
Blood pressure fluctuates, has been "too high" on occassion.
Section B
Have you "always had low or normal blood pressure".
Are you known as "perfectionist" outstanding student in school, come from a high-strung family, sets high standards.
Do you tend to work-off worries, something left undonecauses you unusual concern.
Do you avoid complaints, try to ignore discomforts and inconveniences
Have you had frequent or severe attacks of pneumonia, bronchitis, flu, or sinusitis.

Do emotional storms cause complete exhaustion, must go and lie down when under heavy stress.

Group Three - Metabolic Indicators
Section A
Are your muscles stiff in morning, feel need to limber up, feel "creaky" after sitting for period of time
Feel dizzy or nauseated in morning
Motion sickness when traveling, dizzy when changing up and down positions
Does your heart occasionally seem to "miss beats"
Are following symptoms worse at night, coughing, hoarseness, muscle cramps
Do you have sleeplesness, restlessness, failing memory, forgetfulness
Do you feel better in the afternoon, worse in morning hours
Section B
Do you "go to pieces" easily, dislike working under pressure, cry easily
Gain weight, fail to loose on diets
Long history of chronic constipation
Skin is thick, wrinkly, puffy
Feel better in mornings, worse in afternoon
Difficulty concentrating, easily distracted
On outer third of eyebrows is hair unusually thin or absent
Section C
Does your heart beat above 90 beats per minute when at complete rest

Group Four - Hormone and Enzyme Indicators
Section A
Irregular or discomforting periods

Feel better when resting, lowered endurance, low exercise tolerance
Section B
Have you been treated for heat prostation
Hot weather is unusually discomforting
Ankles swell in hot weather
Ankles swell in afternoon
Prespire excessively in hot weather
Use very little salt
 
Group Six - Epithial Indication
Section A
Pain in lower abdomen, area of bladder
Have had albumin in urine, kidney trouble
Irritated skin, chapped lips, hands, etc.
Skin is rough, dry scaly
Discharge from eyes
Burning or itching when urinating
Swelling of glands in neck
Inability to adjust eyes when entering dark room
 
Group Seven - Carbohydrate Indications
Section A
Nervousness, shaky feeling, headaches are relieved by eating sweets
Irritable if late for meals
Experience sudden strong craving for sweets or alcohol
Get hungry shortly after eating
Often wake up at night feeling hungry
Section B
Night sweets, increased thirst
Chronic fatigue, lowered resistance
History of boils, leg sores, lesions take long time to heal
Overweight
Feel pickup from excersing
At one time was told had suger in urine (Diabetic)
Member of family has diabetes
Crave sweets, but eating sweets makes me feel worse
